Regular Fleshler has been dead since before the first season and is in s01 flashbacks.
In the second season he "regenerated" into an egg. And everyone calls that baby demon that hatches/is born "Lil Fleshler."
Part of the plot is finding out if he will turn out to be the same or not.
In S02e05 there is a small name argument about if his name is "Lil Fleshler" or Scotty. And he's directly addressed as "Lil Fleshler" but also "you're the baby version of someone who [...]."
So they are the same person. But the characters will refer to different versions specifically. It seemed to make sense to nominate it this way. But I could be really wrong!
